Why Would a Root Canal Fail?
Most root canals are successful, but healing can be compromised for several reasons:
- Missed or complex canals – Initial treatment may not have cleaned and filled narrow or curved canals or undetected branches[2].
- Delayed or inadequate restoration – Waiting too long to place a crown or not sealing the tooth properly can allow saliva and bacteria to re‑contaminate the root[3].
- New decay or damage – New cavities can expose the filling to bacteria, while a cracked or loose crown can let infection back in[4].
- Tooth fracture or trauma – A fracture can compromise the seal of the root canal system and permit reinfection[4].
If you experience persistent or recurrent pain, swelling or discomfort in a previously treated tooth, contact your dentist or endodontist. Early intervention offers the best chance of saving the tooth.
Option 1: Root Canal Retreatment
Root canal retreatment is the most common approach to persistent infection. The endodontist will:
- Reopen the tooth – The existing crown or filling may be removed to access the root[5].
- Remove old filling material – The gutta‑percha and other materials inside the canal are taken out[6].
- Clean and examine – Using magnification and illumination, the endodontist searches for hidden canals or unusual anatomy[7].
- Disinfect, fill and seal – After cleaning, the canals are filled and sealed again, and a temporary filling is placed[8].
- Restore the tooth – You’ll return to your dentist for a new crown or permanent restoration to protect the tooth[9].
Pros: Retreatment preserves your natural tooth and avoids surgery. Advances in endodontic technology (microscopes, ultrasonic instruments and cone‑beam CT) may allow your dentist to treat anatomy that wasn’t accessible during the original procedure[10]. Retreated teeth can function for many years or even a lifetime[11].
Cons: Retreatment is more complex than the original root canal; the existing crown, post or core material must often be removed[5]. The cost is typically higher, and some dental insurance policies may limit coverage to one endodontic procedure per tooth[12]. In cases where canals are blocked or the tooth has a post and crown that cannot be safely disassembled, surgery may be more practical[13].
Option 2: Apicoectomy (Endodontic Surgery)
If retreatment is not feasible or has been unsuccessful, an apicoectomy can remove infection from the root end. This minor surgical procedure is performed by an endodontist and involves:
- Local anesthesia and incision – After numbing the area, the surgeon makes a small incision through the gum to access the root tip[14].
- Removing infected tissue and root tip – Inflamed tissue and a few millimeters of the root end are removed, providing direct access to the infection[15].
- Placing a root‑end filling – A small filling is placed in the end of the root canal to seal it and prevent reinfection[16].
- Suturing and healing – The gum is closed with stitches. Bone grafting may be used if infection left a void. Over time, the bone heals and the tooth returns to normal function[17].
Pros: Apicoectomy targets persistent infection when retreatment cannot adequately address the problem (e.g., blocked canals, complex anatomy, or existing posts and crowns)[18]. Modern microsurgical techniques and magnification make the procedure quick and comfortable[19].
Cons: It is an outpatient surgical procedure with minor discomfort and swelling. The success rate depends on factors like the tooth’s anatomy and overall periodontal health. If surgery fails or the tooth is fractured, extraction may be necessary.
Comparing Options and Making the Decision
When deciding between retreatment and apicoectomy, consider:
- Preserving your natural tooth – Whenever possible, it’s best to save your natural tooth[11].Extraction is a last resort because replacement options (implants, bridges or dentures) are more costly and invasive[20].
- Cost and complexity – Retreatment often costs more than the initial root canal because the restoration must be removed and the dentist may need extra time to find hidden canals[21]. An apicoectomy may be recommended if retreatment would require dismantling a crown and post[18].
- Your tooth’s condition – Teeth with cracks extending below the gum line or severe structural damage may not be good candidates for either retreatment or surgery; extraction might be unavoidable. Discuss your specific case with an endodontist.
